Accreditations
Upon completion of the accreditation form, the journalist as a natural person, pursuant to the Regulation (of the European Parliament and of the Council EU) 2016/679 of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (OJ EU L 119 of 04.05.2016, as amended), hereinafter referred to as the “GDPR”, shall express their consent for processing their personal data for the purposes related to the process of granting accreditation and to their participation in the Polish National Arabian Horse Show, Pride of Poland 2021 Auction and Summer Sale 2021.
In view of obtaining your personal data, pursuant to Article 13 of the GDPR, we inform that:
- The data controller, i.e. the entity deciding on the purposes and means of processing your personal data is the Polish Jockey Club (hereinafter referred to as PKWK), with its registered office in Warsaw (02-684), ul. Puławska 266, as the organiser of the Polish National Arabian Horse Show, Pride of Poland 2021 Auction and Summer Sale 2021.
You can contact the data controller using the e-mail address: rzecznik@pkwk.orgor in writing to the address: Polski Klub Wyścigów Konnych, ul. Puławska 266, 02-684 Warsaw. - At PKWK, a Personal Data Protection Inspector has been appointed, who you may contact about protecting and processing your personal data, by e-mail: ochronadanych@pkwk.orgor in writing to the address of our registered office, provided in point 1.
- Your personal data will be processed for the purposes related to granting journalist accreditation for the Polish National Arabian Horse Show, Pride of Poland 2021 Auction and Summer Sale 2021 as well as to confirming your identity/editorial membership and granting you the right to move around the area where the above-mentioned event shall be held and for the purposes of meeting the legal obligation imposed on the data controller, in connection with the Act of 26 January 1984 – Press Law. Your personal data will also be processed for the purposes related to archiving documentation and also for the potential assertion of claims for and against the data controller.
- Your personal data may be made available to other persons or entities, if such an obligation results from the legislation. Your personal data may also be accessed by data processors acting on our behalf, e.g. entities providing services related to organising the event, entities providing legal assistance services, IT services, document shredding services, as well as other personal data controllers processing data on their own behalf, e.g. entities involved in postal or courier activities.
- Your personal data will be processed until the end of the event for which accreditation is conducted and after the end of the event, for the period provided for by the legislation in this regard, including the period of storing the documentation, as stipulated in the common legislation and internal regulations of the data controller on archiving the documents, as well as for the period of prescription of claims for and against the data controller.
- Providing your personal data is voluntary but necessary for obtaining press accreditation for the aforementioned event. Pursuant to the GDPR, you have the right to:
- access your personal data and receive a copy thereof;
- rectify (correct) your personal data;
- limit the processing of your personal data;
- remove your data, in a situation where data processing does not take place for reasons of obligations arising from the legislation.
If you wish to make use of any of these rights, please contact the Personal Data Protection Inspector, as pointed out in point 3, or in writing to the address pointed out in point 1.
Pursuant to the GDPR, each person whose data is processed has the right to submit a complaint to the President of the Personal Data Protection Office.
- Based on your personal data, the data controller will not make any automated decisions on you, including those being a result of profiling.
- The data controller does not envisage providing your personal data to a third country (i.e. a state which is not a member of the European Economic Area, covering the European Union, Norway, Liechtenstein and Iceland) or to international organisations.
- We kindly inform that limitation in granting accreditations for Pride of Poland 2021, Summer Sale and 43rdPolish National Arabian Horse Show covers the number of persons representing individual publishers, editorial offices and titles. Sending a request to grant accreditation also means consent to observe the decisions and statements of the organiser during the event to which the request applies. The organiser shall reserve the right to refuse accreditation.
Wraz z wypełnieniem formularza akredytacyjnego dziennikarz będący osoba fizyczną, zgodnie z przepisami rozporządzenia Parlamentu Europejskiego i Rady (UE) 2016/679 z dnia 27 kwietnia 2016 r. w sprawie ochrony osób fizycznych w związku z przetwarzaniem danych osobowych i w sprawie swobodnego przepływu takich danych oraz uchylenia dyrektywy 95/46/WE (ogólne rozporządzenie o ochronie danych) (Dz. Urz. UE L 119 z 04.05.2016, z późn. zm.), dalej jako „RODO” wyraża zgodę na przetwarzanie jego danych osobowych w celach związanych z procesem udzielenia akredytacji i jego udziału w 43. Narodowym Czempionacie Koni Arabskich Czystej Krwi, Aukcji Pride of Poland 2021 oraz Summer Sale 2021.
W związku z pozyskaniem Pani/Pana danych osobowych, zgodnie z art. 13 RODO informujemy, że:
- Administratorem czyli podmiotem decydującym o celach i środkach przetwarzania Pani/Pana danych osobowych jest Państwowy Klub Wyścigów Konnych (zwany dalej PKWK) z siedzibą w Warszawie (02-684) przy ul. Puławskiej 266 jako organizator Narodowego Czempionatu Koni Arabskich Czystej Krwi, Aukcji Pride of Poland 2021 oraz Summer Sale 2021.
Z Administratorem może się Pani/Pan skontaktować poprzez adres e-mail: rzecznik@pkwk.org lub pisemnie na adres korespondencyjny: Polski Klub Wyścigów Konnych, ul. Puławska 266, 02-684 Warszawa. - W PKWK wyznaczono Inspektora Ochrony Danych Osobowych, z którym może się Pani/Pan skontaktować w sprawach ochrony i przetwarzania swoich danych osobowych pod adresem e-mai: ochronadanych@pkwk.org lub pisemnie na adres naszej siedziby, wskazany w pkt 1.
- Pani/Pana dane osobowe przetwarzane będą w celach związanych przyznaniem akredytacji dziennikarskiej na Narodowy Czempionat Koni Arabskich Czystej Krwi, Aukcję Pride of Poland 2021 oraz Summer Sale 2021, a także potwierdzenia Pani/Pana tożsamości/przynależności redakcyjnej i przyznania Pani/Panu uprawnień do poruszania się po terenie, na którym organizowane będzie ww. wydarzenie oraz w celu wypełnienia obowiązku prawnego ciążącego na administratorze, w związku z ustawą z dnia 26 stycznia 1984 r. Prawo prasowe. Pani/Pana dane osobowe przetwarzane będą także w celach związanych z archiwizacją dokumentacji a także w celu ewentualnego dochodzenia roszczeń przysługujących administratorowi i w stosunku do niego.
- Pani/Pana dane osobowe mogą być udostępniane innym osobom lub podmiotom jeżeli obowiązek taki będzie wynikać z przepisów prawa. Do Pani/Pana danych mogą też mieć dostęp podmioty przetwarzające dane w naszym imieniu, np. podmioty świadczące usługi związane z organizacją wydarzenia, podmioty świadczące pomoc prawną, usługi informatyczne, usługi niszczenia dokumentów, jak również inni administratorzy danych osobowych przetwarzający dane we własnym imieniu, np. podmioty prowadzące działalność pocztową lub kurierską.
- Pani/Pana dane osobowe będą przetwarzane do czasu zakończenia wydarzenia, na które prowadzona jest akredytacja oraz po zakończeniu wydarzenia przez okres przewidziany przepisami prawa w tym zakresie, w tym przez okres przechowywania dokumentacji określony w przepisach powszechnych i uregulowaniach wewnętrznych Administratora w zakresie archiwizacji dokumentów, a także przez okres przedawnienia roszczeń przysługujących Administratorowi i w stosunku do niego.
- Podanie Pani/Pana danych osobowych ma charakter dobrowolny ale jest niezbędne do uzyskania akredytacji prasowej na ww. wydarzenie. Zgodnie z RODO przysługuje Pani/Panu:
- prawo dostępu do swoich danych osobowych oraz otrzymania ich kopii;
- prawo do sprostowania (poprawiania) swoich danych osobowych;
- prawo do ograniczenia przetwarzania danych osobowych;
- prawo do usunięcia danych, w sytuacji, gdy przetwarzanie danych nie następuje w celu wywiązania się z obowiązku wynikającego z przepisu prawa. Jeżeli chce Pani/Pan skorzystać z któregokolwiek z tych uprawnień prosimy o kontakt z Inspektorem Ochrony Danych Osobowych, wskazany w pkt 3 lub pisemnie na adres korespondencyjny, wskazany w pkt 1. Zgodnie z RODO, każdej osobie, której dane są przetwarzane, przysługuje prawo do wniesienia skargi do Prezesa Urzędu Ochrony Danych Osobowych
- W oparciu o Pani/Pana dane osobowe, Administrator nie będzie podejmował wobec Pani/Pana zautomatyzowanych decyzji, w tym decyzji będących wynikiem profilowania.
- Administrator nie przewiduje przekazywania Pani/Pana danych osobowych do państwa trzeciego (tj. państwa, które nie należy do Europejskiego Obszaru Gospodarczego obejmującego Unię Europejską, Norwegię, Liechtenstein i Islandię) ani do organizacji międzynarodowych.
- Uprzejmie informujemy, że ograniczeniu w przydzielaniu akredytacji na: Pride of Poland 2021, Summer Sale oraz 43. Narodowy Pokaz Koni Arabskich Czystej Krwi podlega liczba osób reprezentujących poszczególnych wydawców, redakcje i tytuły. Wysłanie prośby o udzielenie akredytacji oznacza jednocześnie zgodę na stosowanie się do decyzji i komunikatów Organizatora podczas wydarzenia, którego wniosek dotyczy. Organizator zastrzega sobie prawo do odmowy udzielenia akredytacji.
